Enrollment for Grades 1 - 12

All documents and forms needed to enroll students in APS first - twelfth grades are listed below. NOTE: To register students to ride APS buses, please visit the Transportation page.

Documents Required at Registration

  • Your child's Birth Certificate - original or a copy. Birth certificates provided by a hospital or physician are not valid. 
  • Proof of Residency - A utility bill, lease or mortgage is acceptable. We cannot accept auto or voter registration, subscriptions, etc.
  • Complete immunization record or an Immunization Waiver - A non-medical immunization waiver must be obtained from the Ottawa County Department of Public Health (OCDPH). For more information about non-medical immunization waivers call OCDPH at (616) 396-5266. See Immunization requirements and waiver information at right

If you currently have a Parent Portal account through Allendale Public Schools, Please log into your Account and register your new student through Parent Portal.
